5th Community Exposure: Follow-Up Health Monitoring in Barangay Doña Josefa
On February 29, 2024, Team DJ, under the leadership of hypertension CHP leader Aisha Ahmad, embarked on a crucial mission in Barangay Doña Josefa. The day was dedicated to follow-up health monitoring, focusing on two key areas: blood pressure (BP) monitoring and fasting blood sugar (FBS) checking among identified hypertensive individuals and senior citizens.
The team's first priority was conducting follow-up BP monitoring for those previously identified as hypertensive. This initiative aimed to track progress, assess the effectiveness of interventions, and provide personalized care to individuals managing hypertension. The monitoring process ensured that each participant received individualized attention and guidance, reinforcing the importance of consistent blood pressure management.
In addition to BP monitoring, the team also conducted FBS checking for the same group of individuals. This comprehensive approach to health monitoring reflects Team DJ's commitment to addressing hypertension holistically, considering not only blood pressure but also other crucial health markers like blood sugar levels.
